Our person centered care philosophy provides you the opportunity to make the decisions in directing your own care. Our direct care staff works together with you to establish your schedule and care plan.

Care plans are designed in communication with the family and include all disciplines of provided care.

We connect with our residents on an individual basis. It’s about more than just your health and rehabilitation. Your life, interests, and family are equally important to us because that is what is important to you.

We have two full-time Master Social Workers on staff to help support residents and families in all their needs whether they are medical, physical, emotional, or social.

Our social workers work with residents through the adjustments and planning to help make the transition from hospital to skilled facility to home a smooth one.

Our nursing staff is comprised of RNs, LPNs, CNAs, and CMAs. Each of our houses have an RN or LPN on staff twenty-four hours a day 7 days a week. We have nurses experienced in critical care, acute care, and rehabilitation.

We utilize the latest in technology including medical records technologies that keep all records safe and secure on our computer system. Our safe and efficient guidelines and systems ensure your care is the highest quality possible.

Our nursing staff truly cares about you on an individual basis. We strive to exceed the expectation of all our residents.

Nursing care meets the need for care after a hospital stay. Helping patients overcome the challenges of recovering from an injury, illness or surgery. Typical reasons for admission to a skilled nursing community includes:
